自治体マンモグラフィー検診で「構築の乱れ」を疑われた非触知乳癌の二例

We reported the results of screening mammography in lzumo city from April 2005 to March 2006.Acceptance ratio was 1.82%, recall rate was 10%, and the breast cancer detection rate was 1.3%.Among 11 cases detected breast cancer, 7 cases were early cancer, and 3 cases were nonpalpable cancer.
We reported 2 cases of early breast cancer without palpable lesion. They visited our hospital for further examination of their breast after screening mammography with only MLO projection which indicated slight suspicion of "architectural distortion".However, added CC projection showed spiculated mass which suggested malignancy obviously. Two views mammography were helpful to correct diagnosis.
　平成17年度の出雲市マンモグラフィー検診の結果は,受診率1.82%,要精検率10%,癌発見は1.3%であった。発見癌11例のうち早期癌は7例,非触知乳癌は3例であった。
　今回報告した2例の非触知乳癌症例は微細な「構築の乱れ」を疑われ精査機関を受診した。いずれも検診時のMLO撮影のみでは明らかな構築の乱れとは言えなかったが,精査のCC撮影追加で悪性を強く疑いうる症例であり,二方向撮影の重要性を感じた。
