小児科とのコンサルテーション・リエゾン精神医療

総合病院の精神科医は，小児科領域におけるコンサルテーション・リエゾンにおいて，身体疾患に罹患している子どもに生じる精神的な問題や，成長の過程で発症する精神疾患や心身症などへの対応が求められる。小児科と協力・連携して診断，個人および家族精神療法，薬物療法などを行うことになる。子どもの場合は大人と違い，子どもの年齢，性格，認知，感情の発達面を考慮する。今後，小児医療におけるコンサルテーション・リエゾン精神医学の果たす役割は，さらに重要になると思われる。われわれの経験も参考に，連携の実際や考慮すべき点について述べた。

Consultation-liaison psychiatry in pediatric medicine can be defined as the clinical, diagnostic and therapeutic care given by psychiatrists within the general hospital. Consultation-liaison psychiatry often involves children and adolescents who develop psychiatric problem with chronic somatic diseases. We psychiatrists have to carefully consider patient health status, comorbidity, functional impairments and psychiatrically disturbance they experience. Furthermore, child and adolescent psychiatry diagnosis varies widely. The most frequent kind involves adaptation troubles, such as adjustment disorder, depression and psychosomatic disorder. In these cases, there should be consideration ofthe many differences between adult and child psychiatric problems. More comprehensive coordinated care and intervention for the pediatric consultation-liaison should be required in the general hospital.
