地域医療及び総合診療医育成の現状とその向上策に関する質的研究

　Introduction: We discussed about the current situation and development of general
physicians in community medicine in Shimane prefecture, Japan. Through the discussion, we propose a
plan of action.
Methods: We collected data from the discussion, and analyzed the data using the label work
technique. General physicians working in Shimane discussed about community medicine and the
development of general physicians in Shimane on July 20, 2014.
Results: Opinions on the current situation are classified into six categories as follows: medical
practice, learning and training environment, training systems, working environment and support
systems, medical specialty certification systems, and medical students. The suggested action plans can
be classified into six categories: medical student education, standardization of the initial and specialist
training system along with strengthening of the education system, career support, improvement of the
labor environment, interpersonal relationships, and promotional activity.
Conclusion: Many positive opinions were expressed on working and training in rural areas such
as on remote islands and in the mountains. However, some participants indicated problems regarding
medical education and instruction, strengthening of relationships among specialist trainees, and ability
to visualize a longer-term career.

目的：島根の地域医療及び総合診療医育成の現状とその向上策についての議論から、
今後取り組むべきことを提案する。
方法：2014 年７月20 日に島根の総合診療医を対象として島根の地域医療及び総合診療医育成
について議論した。議論から収集されたデータをラベルワーク技法で分析した。
結果：現状についての意見は「診療について」「学習及び研修環境」「研修の教育体制」「勤務
環境と支援体制」「専門医制度に関する問題」「医学生に関すること」の６つのカテゴリに分類さ
れた。今後取り組むべき向上策は、「学生教育」「初期・後期研修の標準化や教育体制の強化」「キャ
リア支援」「就労環境の整備」「交流の場」「啓発活動」の６カテゴリに分類された。
結論：離島や中山間地域での研修や勤務の経験については肯定的な意見が多かった。一方で、
教育に関する課題、専攻医同士の横のつながりの強化、また長期的なキャリアの可視化などの向
上策も挙げられた。
