Middle ear cholesteatoma surgery : our experience 77 cases

Seventy-seven patients with middle ear cholesteatoma underwent a cholesteatoma surgery at Shimane Medical University Hospital, between 1995 and 2002. Out of 77 ears, 42 ears (54.5%) underwent 1-stage operation and 35 ears (45.5%) underwent 2-stage operation. In the 1-stage group, 4 ears (9.5%) had a recurrent cholesteatoma, and in the 2-stage group, 10 ears (28.6%) had a residual cholesteatoma at a time of second stage operation, but no recurrent cholesteatoma was found at all, after the second stage operation, Success rates of hearing improvement, according to the criteria of
Ontological Society of Japan (2000), were 64.3% in children, 42.9% in adults and 47.6% in all.
These results indicate that our middle ear cholesteatoma surgery fairly satisfies patient\s clinical outcome as regard recurrence. But, much more improvement of the ossicular chain reconstruction technique should be needed to increase a success rate of hearing improvement.
In addition, we introduce a rare case of 52-year-old female patient, who had bilateral middle ear cholesteatoma and acoustic neurinoma of the left ear. This patient underwent bilateral ear surgery, and right ear hearing was fortunately improved.
