This study aimed to identify the collaborations and issues between pharmacists working in community pharmacies and home-visit nurses in supporting self-medication of older adults at home. Methods: We conducted semi-structured interviews with eight pharmacists who had more than 5 years of working experience in community pharmacies and who supported older adults at home with self-medication. We asked them about collaborations and issues they encountered with homevisit nurses while supporting older adults at home. The obtained data were analyzed using the steps for coding and theorization method. This study was approved by the Shimane University Nursing Research Ethics Review Committee. Results: Based on the data obtained, a total of 54 themes were extracted for the collaboration between the pharmacists and home nurses and classified into 9 categories. were developed for the collaboration between the pharmacists and home nurses. Pharmacists working in pharmacies reported that they recognized the need for collaboration with home-visit nurses. They also recognized that older adults at home can be supported with self-medication by understanding one another ’s specialties and sharing information. Regarding issues in collaboration, 27 themes were extracted and classified into 5 categories. We found that the pharmacists and home-visit nurses were not fully utilizing one another’s specialties and lacked information sharing. Discussion: To promote collaboration between pharmacists and home-visit nurses, it is necessary to understand one another’s specialties, limits, and cases where they can demonstrate their specialties
本研究は、薬局薬剤師の視点から、在宅高齢療養者の服薬支援における訪問看護師との協働内容とその課題を明らかにすることを目的とした。保険調剤薬局での勤務が5年以上、かつ在宅療養者訪問薬剤管理指導を実施した経験のある薬剤師8 名に半構成的面接を実施し、SCAT分析法にて分析を行った。その結果、協働内容として構成概念・テーマが54抽出され、9カテゴリに分類された。薬局薬剤師は、訪問看護師との協働の必要性を認識し、互いの専門性を理解して情報共有すること等により支援していた。協働における課題として、27構成概念・テーマが抽出され、5カテゴリに分類された。薬局薬剤師と訪問看護師が互いの専門性を十分生かしきれていない現状や情報共有不足等が課題として見出された。薬局薬剤師と訪問看護師の協働を推進するためには、互いの専門性を理解する姿勢、専門性を発揮できる支援内容や支援の限界を相互に理解する必要性が示唆された。
