右側センチネルリンパ節転移を認めなかった陰嚢正中部悪性黒色腫の一例

A 84-year-old man had a scrotal malignant melanoma with left inguinal lymph node metastasis. By lymphoscintigraphy taken after 99m-phytate intradermal injection, positive hot spots were found in bilateral inguinal region, indicating that right spot was sentinel node and left ones were inguinal lymph nodes, Then, we performed wide local excision, right sentinel node biopsy and left pelvic lymph node dissection combined with groin dissection. Metastasis to right sentinel node was assessed by histological, immunohistochemical and RT-PCR examination, and no micrometastasis was found. Thorefore we did not add preventive right inguinal lymph node dissection.

84歳,男性,左鼠径リンパ節に転移がみられた陰嚢正中部悪性黒色腫の症例を経験した。99mTc標識フチン酸によるリンパシンチグラフィーを行ったところ,両側鼠径部に集積を認め,右側はセンチネルリンパ節,左側は鼠径リンパ節への集積と考えた。手術は,腫瘍拡大切除と同時に右鼠径部のセンチネルリンパ節生検および左鼠径甘盤リンパ節郭清を施行した。右側センチネルリンパ節は病理組織学的検査,免疫組織化学的検査,ならびにRT-PCR(reverse transcription-polymerase chain reaction)法を用いたメラノーマ特異抗原による微小転移の検索のいずれにおいても転移がみられず,予防的郭清は不要と判断した。
