がん対策推進計画策定・評価への患者参加のアウトカム評価 : 島根県がん対策推進協議会における委員提案の反映状況分析より

本研究の目的は、島根県がん対策推進協議会における委員提案の島根県がん対策推進計画、同中間評価報告書への反映状況、反映の程度をアウトカム指標として、患者参加が計画策定・評価に及ぼす影響を実証的に検討することである。加えて、委員提案の反映状況に影響を及ぼす要因を分析することである。患者委員提案の計画などへの反映状況、反映の程度は、がん診療連携拠点病院委員などの提案と同等であった。このことから、患者委員は他委員と遜色ない水準でがん対策推進協議会に参加していたと考えられる。委員提案の反映状況に影響を及ぼす委員側の要因として、委員提案の次元の妥当性、提案と計画などの項目・記述との対応関係が示された。患者委員の提案は、他委員とほぼ同様に、がん対策推進協議会における提案として妥当であった。計画などの項目・記述との対応関係についても、他委員とほぼ同様に、留意していた場合と、意図的に留意していない場合があったと考えられる。この点からも、患者委員は、他委員とほぼ同水準の参加をしていたと考えられる。

This paper measures the impact of patient involvement in the development and evaluation of government healthcare policy based on the Shimane Prefectural Cancer Control Promotion Plan. This study analyzed the adoption of committee proposals agreed by Shimane Prefectural Cancer Control Promotion Council, as well as the particular factors of the committee proposals that were adopted.
As a result of the analysis, no significant difference in outcome measures was identified between the patient members representing the patients and the non-patient members (members of a cancer cooperation hospital committee, etc.). The levels of involvement in development and evaluation of the plan by the patient members and non-patient members were considered to be the same. The factors that affected adoption of a proposal were the "validity of the proposal" and "correspondence between the proposal and the item description of the plan". "Correspondence between the proposal and the item description of the plan" was classified into two levels: "good correspondence" or "weak or no correspondence". Between the patient members and non-patient members, there was no significant difference in this factor. From these results, the degrees of involvement by the patient members and nonpatient members were equivalent.
